I have been struggling with a P04DB-Crank case ventilation sensor disconnected- code for the last several months. I have a 2015 Expedition Limited. The tube and sensor have been replaced with OEM Ford parts 2x and the connector is connected. We have found with a smoke test and corrected the leaking seal on the timing chain cover, tried replacing the PCM, and checked for additional leakage in the turbo ducting to no avail. There were secondary codes of Low Turbo Boost-Permanent and Throttle Body Stuck Open-Permanent. Replaced the throttle body and ran the complex "engine cycle". While the CEL is not currently on, the P04DB-Perminate code still appears on the OBD code reader. The others are not still present. I haven't found much about this on the forum or Google searches. Any thoughts would be appreciated.

Is the dip stick going all the way in past the O-ring?
Make sure there is an Oring. IWhen i last changed my oil i noticed the Oring was gone when i went to replace the dip stick. They get old/hard and crack.

I’ll double check the dipstick tomorrow. The shop I took it to did a smoke test. I would assume that would have shown up if it were an issue.

Yes. Generic OBD may show permanent dtc until monitors set. It may not even show up in vehicle specific OBD.
